Measuring effectiveness should address all of the factors that contribute to it.
While a review of this scale might be interesting the cost of conducting it would possibly not release sufficient benefit to justify the activity.
Whatever the criteria, all of the experts agree that the process should be holistic and consider the emergent properties of the system rather than looking at a cause-effect relationship. Finally, no review is worth doing if the resulting reports are not published and the recommendations either acted on or justifiably rejected. The whole reason for being of the review process is to generate learning and a resultant improvement in the management of projects and the supporting processes.
